LHS REQUIREMENTS TO BE A STUDENT-ATHLETE:

1. Online Registration: Parents and students must register and sign all forms in our Final Forms platform.  This should all be taken care of prior to the first practice of the seasonhttps://manitowoc-wi.finalforms.com/

2. Code of Conduct Meeting: Attending at least one code of conduct meeting that is offered through the year. The date for the 2024-2025 year is Monday, August 5th, 6:00-7:30 p.m. in the LHS Auditorium.  

3. Physical Exam: All physical forms (regular or alternate year) must be completed and either uploaded to the final forms portal or submitted to the office before practice can occur. Physicals are generally valid for 2 years.  Alternate year physical forms should be completed on the subsequent year.

4. Concussion Test: must be up to date and completed by all athletes. Our athletic trainers will use the first week of practice to work with teams and perform all necessary concussion testing. If you have any questions contact one of our LHS trainers -stephanie.garcia@aah.org.  

5. Academic Eligibility:  Student earning one or more FAILING "F" OR INCOMPLETE will be suspended from competition for 15 scheduled school days and nights.  This does not include interscholastic scrimmages.  For fall sports/activities, the minimum ineligibility period shall be the lesser of (1) 21 consecutive calendar days beginning with the date of the earliest allowed competition in a sport or (2) One-third of the maximum number of games/meets allowed in a sport. An ineligible student-athlete is still allowed to attend practices and team functions, but not allowed to compete in games/events.
