Sports Registration 
Check List:

Note: The athletic registration and form handling process, including physical exam forms and concussion testing for all MPSD schools are now all online!

Online Sport Registration:

All athletes must be registered on our new online form handling system - Parents MUST create an account for their child BEFORE athletes can access their account. Click here to create a new account or navigate to 'Online Forms - Login' under the Athletics - Forms & Registration tab to create account. All forms and required items will be listed in your personal online forms account. The full list of required athletic forms/waivers are found under the "Forms" tab in the online forms site. 

Code Meeting Video & Questionnaire:
Athletes and their parent/guardian are required to (1) watch the Athletic Code Meeting video and (2) complete the Athletic Code Meeting video questionnaire prior to the start of their sport season - the questionnaire is based on the code meeting video. The video and questionnaire are to be completed in your personal Online Forms Athlete Account
All students and parents must watch the code meeting video then complete the code of conduct questionnaire. The code meeting video is also found below for you reference. 

▢ Physical Exam:
A student-athlete whether an adult or not, must have a physical examination (signed by a licensed physician, physician’s assistant or advanced practice nurse prescriber) every other school year.
A physical examination taken April 1 and thereafter is valid for the following two school years. Physical examination taken before April 1 is valid only for remainder of that school year and the following school year. Click here for the printable Physical Exam Form.

▢ Concussion Test:
Due to the current COVID-19 situation, pre-season baseline ImPACT concussion testing will be completed remotely for the 2020-21 sports seasons. This test is mandatory for all 9th and 11th grade athletes, or any athlete that is new to athletics and does not have a baseline test already on file. This test will need to be complete prior to the first in person practice takes place. To complete a remote baseline ImPACT test, you will need a unique test code sent by Kyle Redo or Steph Garcia LHS Athletic Trainers. Email or to receive a unique “test code” and testing instructions.

▢ Grade Eligibility:
Grade checks are conducted 3 weeks and 1 week prior to the end of each quarter. Athletes are allowed no more than 1 failing grade per quarter to remain eligible for competition. Coaches will be given the list of athletes who have more than 1 failing grade with 3 weeks remaining in the quarter, while parents and students will be contacted with 1 week in the quarter for any athletes still failing more than 1 class.

Athletic Fee:
At this time, athletic fees are not required for athletes to be eligible for practices or competitions. Due to COVID-19, the MPSD may choose to waive the $50 sports fee. If you have already paid this fee for the 2020-21 school year, you will be reimbursed if the district decides to waive the fee.
