Sports Registration

Note: The athletic registration and form handling process, including physical exam forms and concussion testing for all MPSD schools are now all online!

Online Sport Registration:

All athletes must be registered on the online form handling system - Parents MUST create an account for their child BEFORE athletes can access their account. Click here to create a new account or navigate to 'Online Forms - Login' under the Athletics - Forms & Registration tab to create account. All forms and required items will be listed in your personal online forms account. The full list of required athletic forms/waivers are found under the "Forms" tab in the online forms site. 

▢ Attend Code Meeting:
Athletes and their parent/guardian are required to attend the Athletic Code Meeting prior to the start of their sport season.  There will be an IN PERSON code meeting on Monday August 1st at 6pm in the LHS auditorium.  This meeting will be recorded for those unable to make the meeting.  All parents and students must sign off as watched the video in Final Forms.  The video can be accessed in final forms or clicking here.

▢ Physical Exam:
A student-athlete whether an adult or not, must have an updated physical examination (signed by a licensed physician, physician’s assistant or advanced practice nurse prescriber).
A physical examination taken April 1 and thereafter is valid for the following two school years. Physical examination taken before April 1 is valid only for remainder of that school year and the following school year. Click here for the printable Physical Exam Form. This form must be turned into the LHS office or uploaded to the athlete online account. 

▢ Concussion Test:
This test is mandatory for all 9th and 11th grade athletes, or any athlete that is new to athletics and does not have a baseline test already on file. This test will need to be completed before the first practice takes place. To complete a remote baseline ImPACT test, you will need to sign up with our LHS Trainers, Kyle Redo or Steph Garcia LHS Athletic Trainers. Email [email protected] or [email protected] sign up.

▢ Grade Eligibility:
See the LHS Athletic Code of Conduct to see specific requirements on academic eligibility. Student-athletes are ineligible for 15 school days if they earn a failing grade or incomplete (INC) upon each grade mark (end of 1st, 2nd, 3rd, and 4th quarters).

Athletic Fee:
The MPSD requires each athlete to pay a $50 sports fee. This can be paid in cash or check made out to Lincoln High School. If you are unable to pay this fee, please see the main office for a fee waiver form.
