Athletic Code Meetings
Athletes and their parent/guardian are required to attend an Athletic Code Meeting prior to the start of their sport season.  These meetings are held a THREE (3) times in the Fall, ONE (1) in the Winter, and ONE (1) time in the Spring each school year.  Please contact the coach of the sport you are interested in participating in for more information or exact date of the next Code of Conduct Meeting.

Sport Registration Requirements

The following items need to be submitted to the athletic office before an athlete can participate in a sport:

  1. A physical card signed by a licensed physician.
  2. Signed Code of Conduct packet including MPSD student/athletic agreement, understanding and knowledge of the athletic and academic codes, parent/athlete concussion protocol agreement, and the waiver of insurance.
  3. Emergency information card to be returned to the coach of the sport you are participating in.
  4. $50.00 Athletic Participation Fee. This fee is a requirement, per MPSD School Board Policy.
